I don't think the link works but the Maiden next to Big Brown stepped on him coming out of the gate.

what about this one Big Brown photo reveals loose shoe early in Belmont Stakes - Horse Racing - ESPN
I think too many people think its just natural for shoes to fall off when a horse steps on it its no of course every once and while is no big dealt. It should be tight enough to stay on most of the time.
